DO NOT Upload Physicals on 8to18/SNAPManage

Please DO NOT upload your athlete's physical when registering on SNAP Manage.

The Oak Forest Athletic Department is not using this feature and WILL NOT accept an uploaded physical.

Physicals can be emailed as a scanned pdf to Kristin Huie - khuie@bhsd228.com or must be dropped off in the Athletic Office.

Physicals MUST have a stamp from the Dr's office and should have all student & parent signatures completed.  A current completed physical must be on file and school registration must be completed before August 7th in order to tryout/participate in a Fall Sport.
